Wait list only for the Covid Vaccination Clinic Tuesday, April 13th in cooperation with Caring Wellness Pharmacy.  The Moderna vaccine, will be provided by appointment only for Individuals Aged 18+ or Individuals with Pre-existing Conditions (Letter from your Doctor) or Comorbidities (Letter from your Doctor).  

For those who have confirmed appointments, please check your email sent shortly after you made your appointment. To shorten your wait time, please be sure to fill out the necessary paperwork in advance: confirmation of New York State’s COVID-19 Vaccination form and the COVID-19 vaccine consent form. Please bring a government-issued photo ID to your appointment, your Red-White and Blue Medicare card, and/or any other insurance card to your appointment (Including the insurance card you take to your doctor’s office and pharmacy). For those individuals who are unable to complete the paperwork online, there will be forms available at the clinic.

The vaccinations will be performed at the Clifton Park Senior Community Center on the Clifton Common.  The address is 6 Clifton Common Court.  Please arrive five minutes prior to the time of your appointment.  Town staff will greet participants as you enter the facility.
